Later, when a soldier is receiving treatment at the hospital, Owen is prompted to consider joining the US Army by his presence. When he tells Cristina about this, she expresses her disapproval of his choice, saying that she doesn’t want him to pass away, and the two go on to mend their relationship. As the new chief of cardiothoracic surgery, Owen appoints Teddy Altman (Kim Raver), his close friend and coworker from when they were in the army. Hunt and Yang’s relationship is questioned as it becomes clear that Hunt and Altman could be dating, and Yang ultimately decides against it.
Owen gets shot and hurt after a shooting at a hospital, gaining Cristina’s sympathy and leading to the healing of their relationship. In the wake of resident Meredith Grey’s (Ellen Pompeo) interference in Derek Shepherd’s Alzheimer’s trial, Webber resigns, and Owen is elevated to Chief of Surgery. After giving it some serious thinking, Owen unwillingly chooses to go with Cristina to the abortion, bringing the two together. When Owen lies to Teddy about her husband’s sudden death because he wants her to finish an operation she was in the middle of and Teddy blames Owen for her husband’s passing, their friendship comes to an end. Following an argument with Cristina, Owen has a one-night affair with a patient’s friend because he feels abandoned and uneasy. As a result, when Cristina learns, they break up, and their marriage is put to the test. Cristina and Owen make up when she successfully completes her medical boards, but she tells Owen that she is leaving Seattle and moving to the Mayo Clinic. After some time, Teddy and Owen resumed their friendship. She had been given the opportunity to work for the United States Army Medical Command, but out of loyalty, she choose to remain at Seattle Grace Mercy West. Teddy wallows in her grief following the death of her husband Henry, which negatively impacts her coworkers and residents. As a result, Owen “fires” her so that she can start over somewhere else. In addition, he fires Sarah Drew’s April Kepner since the hospital cannot afford to maintain her because she is not Board certified. Hunt revisits Kepner many months later and rehires her after realizing his error. In Season 11, Owen and Amelia Shepherd start dating. He and Amelia get married at the end of Season 12 after she proposes to him, but this goes on and off throughout Seasons 11 and 12. Amelia’s yearning for a “genuine family” and kids draws Owen in. After experiencing a number of marital problems in Season 14, Owen chooses to file for divorce from Amelia. He bases this decision in part on Amelia’s change of heart over having children and her increasingly erratic conduct. When Amelia tells Owen to go be with Teddy in a fit of rage, Owen pays attention to her. As a result, he takes a flight to Germany to be with Teddy. However, after spending the night together, Owen admits that his wife suggested he go. Teddy breaks things off, blaming his inability to be alone for his decision to make her his final resort. Owen arrives at Grey Sloan by himself.
Amelia’s behavior was eventually shown to be brought on by a brain tumor. Amelia is upset with Owen for blaming her alone for the problems in their marriage and for having an affair with Teddy because he dismissed all of her tumor symptoms as selfishness and bad character. They keep having affairs even though they are working on getting divorced. Owen makes the decision to pursue his long-held desire to become a parent alone and starts caring for Leo, a child who was abandoned by his adolescent mother. Amelia takes in Leo’s mother, Betty, who she discovers to be homeless and drug-dependent. Together, Amelia and Owen care for Leo and Betty at Owen’s house, where they reconcile and decide to continue being friends and renew their marriage.
In Season 15, Owen and Amelia have a close bond that is put on hold when it is revealed that Teddy is expecting Owen’s child. Teddy initially held back from telling Owen because she was aware that he would leave Amelia in order to have a child, and Teddy does not want to stay with Owen now that he has made her his second option. Owen is unhappy because if Teddy had told him earlier that he was expecting a biological kid, he would not have made amends with Amelia or fostered Leo. Amelia is even more tense about this because she knows Owen wouldn’t have left her without making an effort to save their marriage. Owen tells Amelia he picks her because Teddy isn’t interested. But because of Owen’s continued possessive and obsessive conduct toward Teddy and her pregnancy, Amelia ended their relationship. Then Owen adopts Leo as his son and welcomes Allison, his new daughter with Teddy, who they named in honor of Allison Brown, Teddy’s best friend who perished in the September 11, 2001, collapse of the second tower. Teddy changes his mind and opts to pursue a romance with Owen.
In Season 16, Owen and Teddy are a couple. He previously gave Teddy his job as Grey-Head Sloan’s of Trauma in order to keep her in the nation. With the new Chief of Surgery, Koracik, whom Teddy dated while she was expecting, Owen has a tense relationship. Koracik files a restraining order against Owen when he unintentionally electrocutes his genitalia, mandating him to stay 500 feet away. Owen makes the decision to take paternity leave in order to allow Teddy to resume her job because she has been finding parenting overwhelming and loathing her maternity leave. After taking paternity leave, he resigns. After finding stay-at-home parenting to be tedious and stressful, Owen eventually accepts Karev’s offer to join Pac-North as Chief of Trauma. However, when Pac-North merged with Grey Sloan, Owen returned to Grey Sloan. After Teddy unintentionally sends Owen a voicemail of her and Koracick having sex, Owen learns that Teddy is cheating on him with Koracick at the end of the season.
After confronting Teddy about her infidelity in Season 17, Owen calls off their engagement. As Owen is reluctant to forgive Teddy, he and Teddy argue at work. However, after she collapses in a catatonic state from overworking herself on the COVID-19 pandemic frontlines, Owen promises to be her buddy. In the season finale, Owen proposes to Teddy, who agrees. Owen and Teddy begin having sexual relations once more.
